How to Register with the Practice

   

It is the policy of this practice to require certain information from patients before registering them with the Practice. This policy is not to decide whether we will or will not register any patient, but merely to ensure that we have sufficient and correct information to offer you appropriate medical services prior to your medical records arriving from your previous practice.

In some cases your notes can take up to 3 months to reach us and should you need urgent medical treatment during this time we will have enough information from your patient questionnaire to enable us to manage your condition safely.

We therefore will not register any patient over 15 until we have a fully completed pre-registration questionnaire. A fully trained member of staff will go through the pre-registration questionnaire with you and enter the information immediately into our clinical system, in this way we do not experience the information gap which can occur when patient’s move practices.

For children under 5 we require the child’s red book to enable us to photocopy the immunisation status of the child to ensure your child is added to the appropriate recall system.

For all patients over 45 we require a blood pressure reading before registration. It will be necessary for all patients over the age of 45 to attend the surgery personally to register.

All patients who are on repeat medication will need to make a doctor’s appointment upon registering.

To be entitled to NHS services you are also required to fill out a GMS1 form, this form will be handed to you as part of your consultation.
