Patient Access connects you to local health services when you need them most. Book GP appointments, order repeat prescriptions and discover local health services for you or your family via your mobile or home computer.

- Order repeat medication
- Book appointments
- View your medical record

How to sign up for Patient Access
Patients 16 and over
To book appointments online, order repeat medications and change your contact information, you will need to complete the Basic Access Consent Form shown below.
Please note: Each patient now requires their own unique email address in order to register for this online service.
You will need to come to the surgery with 2 forms of ID: one with a photo and one with your address (usually a utility bill, bank statement within the last 3 months). We require ID because you are able to view confidential information and this is to ensure you are the patient requesting access.
You will be given a registration sheet by reception including a pin, the practice ID and your Linkage Key. You will need this to register for online access. You will be able to use the service immediately to book appointments and order prescriptions.
Advanced Access
It is now possible to have an advanced level of access that allows you all the features of basic access, but also allows you to have access to your medical record. We currently offer the ability to view
- Previous medical history and other coded information
- Medications
- Immunisations
- Allergies
- Lab results
There are some benefits of having access to your medical records online, but also some risks for you to consider.
Before we can grant you access to this service, we ask you to download and read the information leaflet below. Once you have read the information leaflet, you will need to complete the Advanced Access consent form and return it to the surgery along with ID as for the basic service (unless previously registered for this) to the practice. A GP will need to review your application before granting access and this can take up to 20 working days.
Proxy Access
Parents may find it helpful to have an online account for their children to arrange appointments and order prescriptions. An adult with parental responsibility can apply for an account on their child’s behalf until the age of 11. At 11, online access will be automatically turned off. This is to avoid compromising a young person’s right to confidentiality surrounding their health care as they reach an age when they may wish to see a doctor without a parent present. A child over the age 11 may wish to apply for their own account and should discuss this with their GP. Access to the medical records will not be granted for any patient under 16.
Any patient (or their representative) who does not have an online account or is having problems with Patient Access can order prescriptions via our online prescription service.
If you are the parent of any children under the age of 12 you will need to fill in a ‘proxy’ access form to view their details. You can also grant access to a carer if you wish.

Abuse of Patient Access
Abuse of the system includes, but is not exclusive to the following:
- Sending needless or abusive messages to the Practice
- Repeatedly booking and cancelling appointments
- Repeatedly booking appointments over a short period of time
- Booking appointments and not attending them
- Any offensive or abusive messages through the service
- Requesting prescriptions that you do not need.
Security
We take the security of your information very seriously. Our clinical systems provider ensures that the information you provide when using the online services portal is protected. All communications with the web site are encrypted, using the same high security that major banks use for their internet transactions (128 bit SSL). Only you can see the personal information you enter.
Using the most up-to-date version of your web browser also improves security of information. Remember to close the browser after use, especially if someone else could access your computer.
