Register online
We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

Registration Policy
It is the policy of this practice to require certain information from patients before registering them This policy is not to decide whether we will or will not register any patient, but merely to ensure that we have sufficient and correct information to offer you appropriate medical services prior to your medical records arriving from your previous practice. (Please refer to our registration policy below).
In some cases your notes can take up to 3 months to reach us and should you need urgent medical treatment during this time we will have enough information from your patient questionnaire to enable us to manage your condition safely.
- For children under 5 we require the child’s red book to enable us to photocopy the immunisation status of the child to ensure your child is added to the appropriate recall system
- All patients who are on repeat medication or who have a chronic disease such as diabetes or asthma are advised to have a review with their new GP.
Please be assured that our policy is to enable us to give you the very best medical care and service at all times. All patients, including children will be allocated/informed of their named, accountable GP at the point of registration or at the time of a subsequent interaction with the practice. However if you wish to have this information sooner please contact reception who will be happy to help.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
The National Care Record Service (NCRS)
Important information regarding your NHS medical records – Summary Care Record
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
Online Services
If you wish to register to use our online services (to make appointments and order repeat medications) you will need to bring photographic I.D. to the reception desk.
